Nursing Program Application Instructions
General Instructions
Please complete the following application and ensure that you provide all necessary attachments to satisfy each requirement as listed below. If your application is missing an attachment or the document that you provided does not satisfy the requirement, your application will be determined incomplete and will automatically be declined for admission. Please do not contact the Nursing Department with questions about your application after it has been submitted, as we will not review them until the application deadline. If you recognize you made any errors on your application after it has been submitted, you are welcome to resubmit your application with adjustments and all necessary documents re-attached by the deadline.
Advance review of the materials for your application is highly recommended, and you are welcome to schedule an advising meeting with the nursing department by emailing Jennifer Shenk (Program Specialist II) or calling 928-526-7640. Please be aware that the days prior to the application deadline may not be sufficient time to schedule an advising appointment with the nursing department.
The Nursing Department maintains updated guidelines for each application cycle, which are posted on CCC’s Nursing Program Information page found on the Nursing Program Information Page It is the applicant’s responsibility to utilize these most up to date guidelines for the current application cycle.
Necessary Attachments
The list of attachments below are necessary for your application. If you do not provide the attachment to your application, if the attachment is protected and therefore unable to be accessed by us, or if it does not display sufficient information to meet the requirement, then your application will be considered incomplete.
1. Unofficial Transcripts for Prerequisite Courses
You must attach all transcripts for all prerequisite and corequisite courses from the institution that granted your course credits, displayed with the final grade you received for the course. Do not send official transcripts to us, as they will not be attached to your application. If your school only provides official transcripts, send them to yourself, open them, and scan them as a document attachment to your application. We will request you submit official transcripts if you are admitted to our program.
You are required to have obtained a “C” or better in all prerequisite and corequisite courses. If you received a grade lower than a “C” in any prerequisite course, or if your GPA calculated from the six pre-requisite courses is less than a 3.0, your application will automatically be determined incomplete. If you received lower than a C for any corequisite course, you would not receive the Category #4 points (Total General Education credits completed, form found on the Nursing Program Information Page), if applicable.
2. ATI TEAS Test
Your total TEAS test score must be 66.1% or above and taken within five months of the application deadline. If you provide a TEAS test below that threshold or outside of eligible testing dates, your application will be determined incomplete.
3. DPS Fingerprint Clearance Card
Please provide a copy of the front of your DPS card that displays your name, card number, and expiration date. The card must be active at the time the application is due.
4. LNA License
Verification from the Board of Nursing is sufficient that lists your name, license number, indication that it is “active,” and expiration date. Your LNA license must be active at the time the application is due.
If you have taken the CNA course the semester immediately prior to the application deadline (i.e., Fall semester for March deadline, and Summer semester for September deadline), and your LNA application is currently pending, then you must provide proof of meeting these three requirements:
- Proof that you submitted your application to the Board of Nursing.
- Proof that you have passed your D&S Knowledge and Skills Test. This can either be displayed in your verification of application that you provide from the AZBN (There MUST be a check next to the “test” box to indicate that they received it and that you have met that requirement), or you can provide proof of a passed D&S Knowledge and Skills Test from Headmaster.
- Proof that you have completed your fingerprints for the LNA application. This can be either displayed on your verification of the application that you provide from AZBN (there MUST be a check next to the “fingerprint” box indicating that they have received it and that you have met that requirement), or you can provide a receipt of completion of your fingerprints. If you only provide proof that you have scheduled a fingerprint appointment, or that you have paid for the fingerprinting service without confirmation that you have completed it, it will be considered insufficient proof.
If you only submit proof of some of these requirements, but not all, then your application will be considered incomplete.
Optional Attachments
These attachments are optional as they are not pertinent to every application.
5. Healthcare Experience Verification Form
If you attested “yes” to either employment or volunteer healthcare work, you must provide a letter or form of verification. This form can be found under Forms on the CCC Nursing Program Webpage. You must use this form to satisfy this requirement. If this form is incomplete then it will not be considered as sufficient verification and you would not receive the Category #5 Healthcare criteria points from the Nursing Program Selection Criteria, if applicable.
6. Unofficial Transcripts for Corequisite Courses
You must attach all transcripts for all corequisite courses from the institution that granted your course credits, displayed with the final grade you received for the course.
