You may submit forms to the Registration and Enrollment Services department in person, by mail, via email or fax.

Registration and Enrollment Services Forms:

  1. Add-Drop
  2. Application for Graduation -This form must be completed by an advisor.
  3. Application for Special Admission: Use this form if you have not graduated from high school. 
  4. AGEC Stamp/In-Progress Request
  5. Audit
  6. Change of Grade- This form is for instructors only. 
  7. Change of Major 
  8. Change of Name
  9. Domicile Affidavit-Residency Statement: Use this form if you are under 19 and/or claimed as a dependent
  10. General Petition
  11. Internship Packet- This packet must be completed by student and faculty/dean.
  12. Request for Reprint of Letter of Admission
  13. Satisfactory/Unsatisfactory Form
  14. Student Authorization to Release Information - This form must be completed in-person.
