HEREBY GIVEN that Coconino County Community College District is soliciting bids
from qualified firms for its 4th street restroom remodel. To receive a request for bid document contact Robert Sedillo, Director of

A Mandatory
pre-bid conference is scheduled on Tuesday, March 26, 2013, at 2:00 p.m.
MST, at 3000 4th St., Flagstaff, AZ. 86004.
Bids
shall be sealed and received by the Purchasing Department at Coconino Community
College, 2800 S. Lone Tree Rd., Flagstaff, AZ 86001on or before 2:00 PM, MST on
April 9, 2013.
The
issuance of this request for bid in no way obligates the College District or
State to award a contract. The College
District may, at its discretion, reject any or all proposals, whole; or in
part, and reserves the right to extend, modify or cancel this request for
proposal, at any time, if it is deemed in the best interest of the College
District or State.

NOTICE IS HEREBY GIVEN that
sealed proposals will be received by the Flagstaff Unified School District, Department
of Materials and Contract Management, for furnishing the following items or
services to the Northern Arizona Public Employees Benefit Trust as follows:

Wellness Incentive Program
This RFP is being issued by Flagstaff
Unified School District as the lead agency for the Northern Arizona Public
Employee Benefit Trust (NAPEBT or the Trust) which is comprised of the
Flagstaff Unified School District (FUSD), the City of Flagstaff (COF), Coconino
Community College (CCC), Northern Arizona Intergovernmental Public
Transportation Agency (NAIPTA) and Coconino County. The Trust shall follow the most stringent
requirements of any of the member agencies in the solicitation, evaluation and
any other procedures relating to issuing this RFP.

A pre-proposal conference has

at 2:00 p.m., (Local Time) at the FUSD Administration Building; Board Room; 3285
E. Sparrow Avenue; Flagstaff, AZ 86004.
Although attendance is not
mandatory, Offerors are strongly encouraged to attend. Any doubt as to the requirements of
this Request for Proposal or any apparent omission or discrepancy should be
presented to the Contact Person at that time. The Trust will then determine the
appropriate action necessary, if any, and issue a written amendment to the
request for proposal.
