The "Date Received" is determined by the actual date received in the Security email, not the date you sent it.
If you have e-mail issues you can type in the information and print this form to turn in.
This option may not work if you are using an Apple Computer.
You are not required to sign this form when you email it, only if you print it and turn it into the Security Office.
Before sending this form, open and log into the email program you use, then click the send button on the bottom of the appeal form.
Remember, it is your responsibility to confirm your appeal made it to us. Call us at 226-4304 between 8am and 2pm the following day to see if we have received it.
This form requires that you have a current version of ADOBE READER on your computer, it is free and there is a link on the bottom of this page for it.
If you are attempting to send on the 14th day, it may be in your best interest to print the form and deliver it to the office and it will be date stamped to keep the citation from doubling.
When in doubt, print it and drop it off at Security.
