Internal Customer Forms and Information
Before you fill out the application, below are some other forms you may need to complete and send along with the completed application.
Late Application? | Are you requesting an event that is not within the 14 calendar days? This form can help you get through the process. 2 Week Waiver Request Form |
Fee Waiver | Are you hosting an event and are worried you may need to pay fees? This form can help you find out what fees you may or may not need to pay. Rental Fee Waiver/Reduction Request Form |
Noisy Event | Is your event going to be in the Commons and possibly get a little noisy? If so, we need to remember that it may cause some difficulty for Registration, Financial Aid, and Security to properly do their job so using this form to notify them of such an event will help them adjust properly. Food Handling Certificate | Will you be handling food at your event? If so, we will need a copy of the Food Handlers Certificate. If you don’t have one here is some information regarding achieving that goal. Food Handler Information |
Commons Layout | Are you going to have your event in the Commons and know how you want it to look? Here is the Commons layout that you can draw on. Commons Layout |
Advertise your event! | People at the college, as well as our community outside the college, would love to know what is happening! Please click here to get information on how to do that. |
Facility Use Form | Finally, here is the application. Please fill it out completely, type in your name as your signature and send it to your supervisor. Your supervisor then needs to type in their name, as their signature, and send it to me, Carol Gallegos, via email. |
